In this guide, the defenses against
hazards are classified according to the triggers that instigate them, as follows:
- Functional hazards, associated with the system primary functions
- Hardware fault (such as unit failure, power failure, communication interference ...)
- Automation fault, namely, lost of control when the automatic solution is not appropriate.
- Operational errors (
human fault ):
mistakes,
slips,
omission of required actions
- Situation mismatch (
inter-unit mismatch,
human-machine mismatch,
inconsistent scenario).
- Unexpected hazards, due to specification, design or implementation flaws.
