In Exceptional states

The quality of operation in exceptional states is typically low, because there are many more exceptional states than normal states, and it is easy to sell systems that run seamlessly in normal states. Typically, the quality is developed sporadically, in response to feedback from operators, and to incidences. Often, much of the activities involved in developing this part of the operational procedures are conducted informally, and the development is by trial and error. Consequently, the debugging is lengthy, and the results are of low quality.
