• An alarm may change the operator 's awareness of the
• When under hazard, the situation might escalate, becoming unpredictable
• When in an unpredictable situation, the operation might end up in a mishap (grey swan) or a misfortune (black swan)
• The operation might also terminate as an incidence by a surprising event generated when in routine operation or under a latent hazard .
