Unpredictable situations

Incidences are often associated with unpredictable situations.

Definition

A situation is called unpredictable if the operational mode is unexpected.

Example

An example of common unpredictable situations is of operator-machine mode mismatch. In particular, situations when the human operator is not aware of a change in the machine situation are most common.

Sources of unpredictable situations

Unpredictable situations result from missing or wrong specifications, design mistakes, or implementation errors (mainly, software bugs).

Inconsistent states are perceived as unpredictable, which means that the operators typically cannot identify the source for the ambiguity.
