Following an incidence or an accident, the stakeholders typically focus on accountability issues, looking for "bad apples" ( Dekker, 2007 ) instead of on improving the safety.
Emotion-driven response to incidences hampers the efforts to learn from incidences, because the investigations focus on the stakeholders instead of the design changes needed to improving the resilience.
On the other hand, when the organization adopts ergonomic culture, the investigations include recommendations for design changes, and the management promotes implementing these recommendations.
