Escalation is the term used to describe the transition from an exceptional
situation to a
situation with which the
operator is not familiar.
Escalation happens when the
operators do not operate according to the rules
suited to the exceptional
situation.
Failure modes
- The operator is not aware of the
system being in an exceptional
situation, and operates according to procedures proper to normal situations
- The operator is aware of the
system being in
exceptional situation, but
is not familiar with the proper
operational procedures.
