Risks of manual control

Manual operation might fail:

• When the manual operation requires fast, complicated calculations.
• In extreme conditions.
• When the proper operation is different from the one to which the operators are accustomed (see control paradox ).

Human performance is limited

Manual operation might be too slow, or imprecise. Examples:

• In the Soyuz 11 accident, the astronauts could not complete the problem solving in the short grace time they had.
• A machine controlled manually  ...
• Slip-prone switch ...
• Wrong setting of an alarm clock ...

Risks about operating an improper interface

  A main source for operational error is due to applying a standard, consistent interface, to operate both in routine and emergency.

The risks are about the operator's failure to understand the situation and to respond appropriately.
