Sources of detection faults

Detection failure may result from:

• Missing dedicated fault detector: for example, if a container is not equipped with a means to detect leakage
• Reliability deficiency of a resilience add-ons

Example

• The PORV of the the Davis Besse 1 incidence was due to the missing of a dedicated fault detector
• The PORV of the TMI incidence was due to a reliability issue
