Many incidences result from scenario ambiguity, due to implicit mode setting.
The problem is that the operators do not always follow the predictions specified at the design time. Subsequently, during operation, there are situations of exceptional machine state, which the operators sometimes fail to recognize. When the operational scenario is implicit, the machine may not be able to identify situations of unexpected operator intentions. In such cases, the machine might not respond properly to the operators commands, resulting in cooperation fault.
During the operation, the operational scenario is implicit, and the machine does not know the operator's intention. The operational scenario is not specified, and there is no way for the machine to find out which of the possible scenarios is active. Many legacy systems implement algorithms for predicting the operators intention based on the history of the operators commands.
