Human-machine
mismatch is a special case of
inter-unit mismatch, when at
least one of the units is a human
operator.

Case studies
- 1967 Torrey Canyon Supertanker disaster. The
operator was not aware of the
navigation in
control mode, in which the steering rod was disconnected from the
wheel at the helm
- 1979 TMI nuclear power plant accident. The
operators were not aware of the backup pumps being closed following a maintenance procedure.
The problem of operator-machine cooperation
This problem is realized in
mode confusion, when the
operators fail to recognize the
machine state, or when the machine
behavior does not suit the
operator's intentions.
