A fault is an event that hampers the normal operation.

A critical fault is defined as a fault that ends up in an incidence.

Location of faults

Faults may be classified according to their location, as follows:

• Hardware faults, occur in the machine units
• Operators' faults, unintentional actions (slips) or mistaken intentional actions, due to operator-machine (scenario-situation) mismatch
• Inter-unit coordination fault (situation mismatch)

Risks of faults

Following a fault, the system enters an exceptional situation. Operators typically have little or no opportunity to learn how to control rare situations, and are most likely to fail.
