Alarm generation
Alarms may be generated either
automatically, following detection of an
exceptional situation by
sensors, or
manually, by the
operators reporting about a
hazard.
Operator generated alarms
Operators occasionally fail in
alarm generation, in various situations:
- Inadequate training: for example, when the
operators are used to rely on other sources of information (such as
hazard
indicators).
- Conflicting information: for example, when the information received from other sources is misleading (as was the case of the TMI PORV
state indicator)
- Hesitation: for example, when the
operators are not sure about the
situation, and the are afraid of being accused for creating unnecessary panic (as is often the case with war
alarms).
Default alarming
Whenever possible, by default, the
alarms should be generated automatically, by alarm trips.
