Clearly, the primary
hazard (navigating in the direction of the reef) was detected and identified in time.
The problem was with the secondary
hazard, which was latent. The ship did not provide direct indication about the secondary
hazard. The captain became aware of it only indirectly, by the lack of clicks from the gyro, which were expected in case of changing the ship's route.
The guide proposes that
alarms are specified for all potential hazard, in a way that the operators can recognize the risky
situation in time.
