The designed enabled the operator to perform risky operations. An error-proof design should disable performing routine risky maintenance activities during production

Perception

The operators were misled by the wrong indication of the state of the PORV. Also, they were confused by the indication of low pressure, which hinted on the system operating with sufficient coolants. Consequently, they turned off the auxiliary pumps, which had been activated automatically. This action turned out to be fatal.
